Dietary Forms for Meal Modifications should be completed by a licensed medical authority and then returned to the nursing staff at your student's school. The form will be forwarded to the School Nutrition Program. This process will take about a week or more to be implemented and to appear in student records. The safest meal for dietary modifications is always one prepared at home. Dietary modifications can only be made for food allergies documented by a licensed medical authority.
